Certificate of Rehabilitation and Pardon

Is there an official certificate of rehabilitation form for
Loa Angeles County California or is there one generic form?  Can someone please direct me to the
link?  Are there examples of how to
complete this form?  Thanks so much!  This is a great site!

Sorry for the slow response.  

Los Angeles uses the standard CA Certificate of Rehabilitation form.

All certificates of rehabiliation are filed at the Los Angeles Criminal Justice Center at:

Los Angeles Criminal Justice Center

California Superior Court 

210 West Temple Street, 5th flr. Rm. 5-305

Los Angeles, CA 90012

Phone Hours: 8:30 – 10: 30 and 1:30 – 3:30

213-974-6141 Front Desk

The process can take about 3 to 4 months and it requires a lot of interaction and investigation from the District Attorney.

I believe the LA Public Defender's office can provide you a packet for Los Angeles.

That link above provides good information for people who reside in LA and who cannot afford an attorney… or meet the low-income requirements of the Pubilc Defender's office.

Go right to the source for Help & info. No walk ins are accepted. Call 213-974-3057 more than likely you will have to leave a message, but he will get back to you and explain it all & set his is the person designated by the Los Angeles County public defenders office to handle this. He will instruct you what to obtain then come in to sign your forms for submission. NO COST.
